Referrals

The Universal aspects of our service do not require a referral, but we must recieve a completed referral form with parental consent before we can offer targetted support. 

Who is eligable for Targetted Autism Outreach?

We can accept referrals for school-age pupils who attend Surrey schools and academies.

Most of the pupils that we support will have a diagnosis of autism, but some may not and a referral is still appropriate for pupils with similar needs, or those who are awaiting diagnosis.

No EHCP is required for a referral 

How to send a referral? 

Click below to find the referral from for your area/phase and the address to send it to. The forms will need to be emailed to us, using an Egress account, although we can accept referrals sent via standard email from @surrey.sch.uk email addresses.

Traded Referrals 

Independent schools, or those outside of the Surrey borders can access AOFS as a traded service. The cost is £275 for a half-day visit from an Outreach worker, including a classroom observation, feedback meetings for staff and parents with a full written report. To make a traded referral, please complete the form from the most appropriate team above and the they will be in contact to discuss the process.
